Passport applications are accepted on a first come, first served basis.

Valid Driver’s License, State Identification
Card, Employee ID, or Military ID
Your Original Birth Certificate or Naturalization Certificate
(Certificate will be returned to address on your DS-11 application)
PASSPORT FEES
Check, or money order (No Cash) made out to the U.S. Department of State
will be accepted in the amount of
$110 for adults and $80 for children, and a $35 fee by
check or money order made out to Norwalk Public Library.
