The Norwalk Public Library has become an official Passport Application Acceptance Facility. U. S. Passport applications are accepted on a first come, first served basis.
1st Time Applicants
Lost/ Stolen Passports
Adult renewals ONLY if passport is expired 5 years or more.
Bring a completed DS-11 Application Form
A recent Passport Photo-2X2
Valid Driver's License, State ID Card, Employee ID or Military ID
Your original Birth Certificate or Naturalization Certificate (Certificate will be returned to address on your DS-11 application)
Check or money order (NO CASH OR CARDS) made out to the U.S. Department of State will be accepted in the amount of $110.00 for adults and $80.00 for children, and a $35.00 fee by SEPARATE check or money order made out to Norwalk Public Library.
